Hometown Health is a customized employee wellness solution developed to help Florida Municipal Insurance Trust members with group health insurance coverage save money.
Hometown Health offers an incentive program where FMIT members can participate throughout the year. The incentive program starts off with onsite bio-metric screening events and is followed immediately by an online Health Assessment through our Hometown Health portal, which is secure and password protected.
In addition, Hometown Health offers a second level incentive program (the reward is larger if this level is completed) for their members. This level of incentive is more involved and really works on making lifestyle changes for the individual, which is done through telephonic coaching. Hometown Health also offers Wellness Challenges throughout the year to our members as well and is another component to the second level incentive program.

The Florida Municipal Insurance Trust offers several different benefit plans for its members through UnitedHealthcare. If you are a current member of the FMIT with coverage through one of these plans, you can register to access current and comprehensive information regarding your benefits. You can log on at the link above, click "Register Now" and enter the requested information and begin using the site. Once you are registered, you will be able to find hospitals and physicians in your area, check claim status, view claim history, request a medical ID card and find many other ways to make the most of your benefits. You can also call UnitedHealthcare at (866) 633-2446 and speak directly with a representative.
The FMIT provides two dental plan options through UnitedHealthcare with annual maximum coverage limits. Both plans include benefits for the following services:
Preventative & Basic Care
Oral Examinations, Dental X-rays, Dental Cleanings, Fillings, Extractions, Fluoride Treatments (children under age 15)
Restorative & Specialty Care
Oral Surgery, Endodontics, Periodontics, Crowns/Bridges/Inlays/Dentures, Orthodontics (children under age 19)
The FMIT's Vision Plan, provided through UnitedHealthcare, offers coverage for the following services provided by participating professional UnitedHealthcare providers:
Eye examination and refraction, lenses - single vision, bifocal, trifocal, frames - Wide selection available of medically necessary contact lenses.
The FMIT offers Group Term Life Insurance coverage through ReliaStar Life Insurance Company. ReliaStar is an affiliate of the well-known finance and investment group ING. This benefit offers the following advantages: accidental death and dismemberment (included in coverage), portable benefit, flexible group coverage and affordable rates.
