Medical
The Florida Municipal Insurance Trust offers several different benefit plans for its members through United Healthcare. If you are a current member of the FMIT with coverage through one of these plans, you can register to access current, comprehensive information regarding your benefits. Just log on and click "Register Now" and enter the requested information and begin using the site. Once you are registered, you will be able to find hospitals and physicians in your area, check claim status, claim history, request a medical ID card and find many other ways to make the most of your benefits. You can also call United Healthcare at 1-866-633-2446 and speak directly with a representative.
Dental
The FMIT provides two dental plan options through United Healthcare with alternative annual maximum coverage limits. Both plans include benefits for the following services: Preventative & Basic Care - Oral Examinations
- Dental X-rays
- Dental Cleanings
- Fillings
- Extractions
- Fluoride Treatments (children under age 15)
Restorative & Specialty Care
- Oral Surgery
- Endodontics
- Periodontics
- Crowns/Bridges/Inlays/Dentures
- Orthodontics (children under age 19)
Vision
The FMIT’s Vision Plan, provided through United Healthcare, offers coverage for the following services provided by participating professional United Healthcare providers:
- Eye Examination and Refraction
- Lenses – Single Vision, Bifocal, Trifocal
- Frames – Wide Selection Available
- Medically Necessary Contact Lenses
Term Life
The FMIT offers Group Term Life Insurance coverage through ReliaStar Life Insurance Company. ReliaStar is an affiliate of the well-known finance and investment group ING. This benefit offers the following advantages:
- Accidental Death and Dismemberment (included in coverage)
- Portable Benefit
- Flexible Group Coverage
- Affordable Rates
