The Special Investigation Unit (SIU) was created in early 1999 to reduce the cost and amount of fraud to the Floirda Municipal Incurance Trust (FMIT) members. The SIU gathers information of possible fraudulent insurance acts, potential abuses, or non-existent injuries. Suspected fraud is reported to the Florida Department of Financial Services, Division of Insurance Fraud. Remember, fraud is not a victimless crime. Agencies that must pay for fraudulent insurance claims also have higher rates and less money for other expenditures.

Please provide as much information as possible to allow a complete and thorough investigation.
